Consent Form
This research is being conducted by Joseph Hammer, who is under the supervision of Dr. R. Chris Fraley at the University of Illinois. This is a Health Evaluation Study, designed specifically for men. We will ask you to read a brochure and answer a series of relevant questions. The study will take no longer than 12 minutes. You must be 18 years of age or older to participate.

You will not be subject to any significant risks in this experiment. Potential benefits to you include gaining some knowledge of how psychological research is conducted as well as contributing to the advancement of scientific knowledge. The results of this study may be presented at conferences or published in journal articles in the future. Your name or other personally identifying information will not appear anywhere in the materials we collect from you, so complete anonymity is assured.
